Unisom vs Melatonin Is a Sedative Against a Timing Signal

Medically reviewed by Dr. Michael Teplitsky, MD Last reviewed 2026-10-21

Doxylamine and diphenhydramine carry heavier label warnings than melatonin does.

Unisom is a sedating antihistamine with heavier warnings and a two-week label limit. Melatonin is a weak timing signal that fits jet lag and a late body clock better than ordinary insomnia.

Unisom is the stronger sedative of the two. At The Metabolic Journal, the mix-up we see most often is treating Unisom and melatonin as a strong and a weak version of one pill.

Unisom SleepTabs contain doxylamine, a sedating antihistamine whose label says to stop and see a doctor if sleeplessness lasts more than two weeks. Melatonin is the hormone your brain makes in response to darkness. It helps most when your body clock is off, as with jet lag, and does little to hold you asleep.

Unisom vs Melatonin Strength for Falling Asleep

Unisom is stronger than melatonin as a sedative, but neither one shortened sleep by much in sleep-lab trials. The American Academy of Sleep Medicine (AASM) 2017 insomnia guideline found that diphenhydramine 50 mg, the drug in Unisom SleepGels, cut the time to fall asleep by about 8 minutes. Melatonin 2 mg cut it by about 9 minutes.

The AASM panel set 10 minutes as the smallest change that matters, so both fell short on the clock. The bigger difference shows up in the warnings. The SleepTabs label tells you not to drive or use machinery after a dose, while the AASM found no melatonin harms that stood out from placebo.

Doxylamine, the drug in SleepTabs, was not among the 14 medicines the AASM reviewed. No source we found has tested doxylamine head to head against melatonin. Unisom's extra strength shows up in the SleepTabs label instead, which rules out driving and alcohol after a dose and use without a full night of sleep.

The brand name does not tell you the drug. The Unisom line includes both a doxylamine tablet and diphenhydramine softgels, and ZzzQuil LiquiCaps use diphenhydramine, two 25 mg capsules per dose. The DailyMed listing for ZzzQuil shows that ZzzQuil Ultra contains doxylamine instead, so read the active ingredient line on the box before you compare anything.

Unisom vs Melatonin Side by Side

Unisom and melatonin differ most on next-day effects, older adults and how long you can keep taking them.

Older adultsBeers Criteria say avoidSame dose gives higher blood levels, and AASM advises against it in dementia
PregnancyAsk a clinician first. Doxylamine 10 mg is part of a prescription pregnancy nausea drugNHS generally does not recommend it
ChildrenNot for children under 12Only with a pediatric clinician's advice
VerdictAn occasional night of trouble falling asleep, for a healthy younger adultJet lag and a body clock that runs late

The American Geriatrics Society (AGS) 2023 Beers Criteria list both diphenhydramine and doxylamine as drugs older adults should avoid. The AGS panel calls them highly anticholinergic, with risks of confusion, dry mouth and constipation. It links cumulative use of anticholinergic drugs to falls, delirium and dementia, even in younger adults.

The pregnancy row needs care. Doxylamine 10 mg is one half of Diclegis, a prescription tablet for nausea and vomiting of pregnancy, and its label reports no increased risk of birth defects in epidemiologic studies.

That evidence covers a lower dose taken for nausea under a doctor's care. It is not a green light for Unisom as a pregnancy sleep aid without asking your obstetric clinician.

Next-Day Grogginess and Nightly Use

Unisom is more likely than melatonin to leave you foggy the next morning, and neither one is built for months of nightly use. The SleepTabs label rules out taking it unless you can get a full night's sleep, and warns against alcohol. The Beers Criteria add that tolerance develops when these antihistamines are used as sleeping pills, so the same dose tends to do less over time.

Melatonin clears fast. A 2015 pharmacokinetic review put its half-life at about 45 minutes. A 2023 review of melatonin in older adults found next-day sedation mostly follows daytime dosing, and suggests taking it 30 to 60 minutes before bed.

Both have a time limit in practice. The Unisom labels tell you to see a doctor after two weeks of sleeplessness, and the UK's National Health Service (NHS) prescribes melatonin for 1 to 4 weeks. Our melatonin side effects guide covers what is known about longer use, including a 2025 heart failure abstract.

When to Choose Unisom

Choose Unisom for a single rough night when you are a healthy adult with time for a full night in bed. Plan on not driving the next morning.

Take it the way the label says. SleepTabs call for one 25 mg tablet 30 minutes before bed, once a day, and SleepGels for one 50 mg softgel at bedtime if needed. ZzzQuil LiquiCaps limit you to one dose in 24 hours, so a second dose at 3 am goes against the label.

Skip Unisom if any of these apply, because the labels tell you to ask a doctor first:

  • glaucoma
  • trouble urinating from an enlarged prostate
  • emphysema, chronic bronchitis or another breathing problem
  • you take sedatives, tranquilizers or other sleep aids

Do not double up on the same drug. The labels warn against taking SleepTabs with any other doxylamine product, or SleepGels with any other diphenhydramine product, including products you put on your skin. Combination products such as Unisom PM Pain and ZzzQuil Night Pain also contain diphenhydramine, per the DailyMed listing for Unisom, so check every box in your cabinet.

When to Choose Melatonin

Choose melatonin when the problem is timing. The Cochrane review of jet lag recommends it for adults crossing five or more time zones, especially flying east, at 0.5 to 5 mg taken at bedtime after arrival.

For a body clock that runs late, the AASM 2015 circadian guideline weakly supports timed melatonin, based on adult trials of 5 mg taken between 7 and 9 pm. The National Center for Complementary and Integrative Health (NCCIH) describes a trial where a dose 1 hour before a fixed bedtime moved sleep onset 34 minutes earlier.

Shift workers get less help. NCCIH reports that one review of 7 studies found about 24 extra minutes of daytime sleep with melatonin, and rated that evidence low quality.

Melatonin is the weaker pick for waking at 2 or 3 am. In the AASM data, 2 mg did not cut the time spent awake after first falling asleep. Our melatonin dosage guide has doses and timing for each situation.

Who Should Choose Neither

Some people should choose neither Unisom nor melatonin. Insomnia that hits at least 3 nights a week for 3 months is chronic.

For chronic insomnia, the American College of Physicians (ACP) recommends cognitive behavioral therapy for insomnia (CBT-I) as the first treatment. Our insomnia guide explains how CBT-I works and which prescription drugs come after it.

Loud snoring, gasping at night or heavy daytime sleepiness need a sleep apnea check before any sleep aid. ACP lists sleep apnea among the causes to rule out, and our sleep apnea vs insomnia comparison shows how to tell them apart.

Other groups who should ask a clinician before either one:

  • older adults or anyone with memory problems, given the Beers warning on Unisom and the AASM warning on melatonin in dementia
  • pregnant people
  • children, since Unisom is not for children under 12 and the AASM advisory asks parents to see a pediatric clinician before melatonin
  • anyone on warfarin, which the NHS lists as a melatonin interaction

What Would Change Our Answer

Our verdict would change with three kinds of new data. A head-to-head trial of doxylamine against melatonin would settle the strength question with numbers. An AASM review that included doxylamine would tell us whether it beats diphenhydramine's small effect.

A peer-reviewed version of the American Heart Association (AHA) melatonin abstract that confirms the heart failure link would also move us. In that case, melatonin would lose its edge for anyone using it nightly for a year or more.

If you need something stronger than either, a prescription comparison such as trazodone vs Ambien is the next step to discuss with a clinician. For a non-drug option, read magnesium for sleep, and see the full sleep and recovery hub. If you are choosing Unisom vs melatonin tonight, read the active ingredient line first, then match it to the problem in the table above.

Frequently Asked Questions

Is Unisom or melatonin safer?

Melatonin is safer for most adults over a few weeks. AASM trials found no harms from melatonin that stood out from placebo, while the Beers Criteria tell older adults to avoid the antihistamines in Unisom. Long-term melatonin safety is still unproven.

Is Unisom and melatonin the same thing?

No. Unisom is a brand of antihistamine sleep aids containing doxylamine or diphenhydramine. Melatonin is a hormone sold as a supplement.

Can Unisom and melatonin be taken together?

Ask a pharmacist first. The SleepTabs label tells you to check with a doctor or pharmacist before use if you take other sleep aids, and melatonin counts as one.

Is Unisom or melatonin better for pregnancy?

Neither should be used for sleep in pregnancy without your obstetric clinician's advice. The doxylamine evidence in pregnancy comes from the 10 mg Diclegis tablet prescribed for nausea, and the NHS generally does not recommend melatonin while pregnant, per its pregnancy guidance.

Is ZzzQuil stronger than melatonin?

ZzzQuil is the stronger sedative. Standard ZzzQuil LiquiCaps give 50 mg of diphenhydramine per dose, the same drug and dose as Unisom SleepGels. In AASM sleep-lab trials, 50 mg of diphenhydramine cut the time to fall asleep by about 8 minutes. The ZzzQuil label also tells you to stop and see a doctor if sleeplessness lasts more than two weeks.

Is Unisom or melatonin better for staying asleep?

Neither has good evidence for staying asleep. The SleepTabs label lists only reduced difficulty falling asleep as its use, and the AASM had no wake-time data for diphenhydramine and no benefit for melatonin. The AASM 2017 guideline weakly supports doxepin and suvorexant for staying asleep, two prescription drugs a clinician can weigh with you.

What is a good alternative to melatonin?

For chronic insomnia, the best-supported alternative is CBT-I, which ACP recommends before any pill. If a medicine is still needed, the AASM 2017 guideline weakly supports eight prescription drugs, including zolpidem, eszopiclone, ramelteon and doxepin. Our melatonin dosage guide covers when melatonin itself is worth a try.

Can I take Unisom every night?

Not for long. The Unisom labels say to stop and see a doctor if sleeplessness lasts more than two weeks. The Beers Criteria also note that tolerance develops when these antihistamines are used as sleeping pills.
