What the 2026 Cholesterol Guidelines Mean for Your Numbers
The ranges above show what is normal, but the 2026 ACC/AHA dyslipidemia guideline changed how doctors turn those numbers into a plan. It replaced the 2018 cholesterol guideline and restored specific LDL goals based on your risk: under 100 mg/dL for most adults, under 70 mg/dL if you have risk factors or a prior heart event, and under 55 mg/dL if you are very high risk. It also recommends that every adult measure their lipoprotein(a), or Lp(a), once in their lifetime — an inherited particle that raises heart risk on its own, separate from LDL. And it formally recommends losing at least 5% of your body weight if you carry extra weight, because that improves triglycerides, HDL, and LDL particle size together. Cholesterol Levels in mmol/L: UK, Canada, and Australia
Every chart on this page uses milligrams per deciliter (mg/dL), the standard US unit. The UK, Canada, Australia, and most of Europe report cholesterol in millimoles per liter (mmol/L) instead — a much smaller-looking number that confuses a lot of readers comparing results internationally. To convert, divide mg/dL by 38.67 to get mmol/L for total cholesterol, LDL, or HDL (for triglycerides, divide mg/dL by 88.57 instead, since triglyceride molecules are a different size). To go the other way, multiply mmol/L by 38.67 (or by 88.57 for triglycerides).

That rate-limiting step is regulated by luteinizing hormone (LH) and catalyzed by the enzyme cytochrome P450 family 11 subfamily A member 1 (CYP11A1), before the testes complete production through several additional enzymatic steps. Your body requires adequate cholesterol to maintain this pathway, but clinically low testosterone caused by low cholesterol alone is unusual in people with typical or low-normal lipid numbers. The target follows a simple rule: non-HDL goal equals your LDL goal plus 30 mg/dL. So if your LDL target is under 100 mg/dL, your non-HDL target is under 130 mg/dL; a target of 70 mg/dL becomes 100 mg/dL non-HDL, and 55 mg/dL becomes 85 mg/dL. Unlike LDL, non-HDL cholesterol doesn't require a fasting blood draw to measure accurately, which is part of why it's gained favor for routine screening.
What is the difference between LDL cholesterol calculated and LDL direct?
Most lab reports list LDL cholesterol as "calculated" rather than directly measured. Labs use the Friedewald equation — LDL = Total cholesterol − HDL − (Triglycerides ÷ 5) — to estimate LDL from your other three lipid values, since directly measuring LDL used to be expensive and slow. This calculation works well for most people, but it becomes unreliable once triglycerides climb above 400 mg/dL, or if the blood draw wasn't fasting, because the formula assumes a fixed ratio between triglycerides and VLDL cholesterol that breaks down at high triglyceride levels — in that range, calculated LDL tends to underestimate the true value. "LDL direct" is a separate lab method that measures LDL particles directly rather than deriving them from the other three numbers; it costs more and isn't run by default, but it's the more accurate option when triglycerides are high, the sample is non-fasting, or in someone with diabetes or metabolic syndrome, where elevated triglycerides are common. If your report says "LDL Calc" or "LDL Cholesterol Calc," that's the Friedewald estimate; "LDL Direct" means it was actually measured.
Is high cholesterol the same as hyperlipidemia?
Not exactly, though the terms overlap heavily. Hyperlipidemia is the clinical umbrella term for any elevated blood lipid — cholesterol, triglycerides, or both — and it's the diagnosis code your doctor is more likely to actually use. "High cholesterol" in everyday language usually refers to one specific piece of that picture: elevated LDL or total cholesterol. Since hyperlipidemia can also be driven by high triglycerides alone with normal cholesterol, someone can have hyperlipidemia without technically having "high cholesterol," which is why the lipid panel breaks these values out separately rather than reporting a single number.
Are there alternatives to statins for high cholesterol?
Yes — several non-statin, FDA-approved options exist, and a doctor may consider them alongside or instead of a statin depending on the situation, not as a reason to avoid statins altogether. Ezetimibe lowers LDL by blocking cholesterol absorption in the gut and is often added to a statin or used alone if statins aren't tolerated. PCSK9 inhibitors — injectable antibody drugs including evolocumab and alirocumab — produce larger LDL reductions and are FDA-approved for people with familial hypercholesterolemia or existing cardiovascular disease who need additional lowering beyond a statin. What is a good cholesterol ratio (total cholesterol to HDL)?
Your cholesterol ratio — total cholesterol divided by HDL — is a separate metric from any single number above, and some clinicians use it as a quick cross-check. According to Mayo Clinic, most people should aim for a ratio below 5:1, with under 3.5:1 considered especially favorable; since HDL targets differ by sex (≥40 mg/dL for men, ≥50 mg/dL for women), the ratio a man and a woman need to hit the same target number can differ. To calculate it, divide your total cholesterol by your HDL from a recent lipid panel — a total of 200 with an HDL of 50 gives a ratio of 4. Mayo Clinic notes this ratio has largely been supplanted by non-HDL cholesterol and direct LDL goals (see above) as the primary treatment target in current guidelines — it's still a useful quick read, but your clinician is more likely to focus on your LDL and non-HDL numbers when deciding on treatment.
