High cholesterol is one of the most common health conditions in the United States, and one of the most misunderstood — because unlike most conditions people search symptoms for, it almost never produces a symptom you can feel. The things people often associate with "high cholesterol" — fatigue, headaches, chest tightness — are either myths or, when they're real, are actually symptoms of a downstream disease process (atherosclerosis) that can develop after years of elevated cholesterol, not of the elevated lab number itself.

Why High Cholesterol Usually Has No Symptoms

High cholesterol is often called a "silent" condition, and that description is clinically accurate: for the large majority of people, elevated LDL, total cholesterol, or triglycerides produce no headache, no fatigue, no chest tightness, and no other physical sensation at all. The CDC states plainly that high cholesterol has no symptoms, which is exactly why so many people don't find out their numbers are elevated until a routine blood test — sometimes years or decades after cholesterol first started climbing.

This matters because cholesterol builds plaque inside artery walls (atherosclerosis) gradually and silently. Arteries don't have pain receptors that signal narrowing in progress, so plaque can build up substantially before it produces anything you can feel. It's also why high cholesterol is sometimes discovered only after a heart attack or stroke — the first noticeable event is the downstream complication, not the cholesterol problem that caused it. There are two real exceptions to the "no symptoms" rule, covered below: visible deposits in skin and around the eyes, and symptoms that appear only after cholesterol has already caused significant arterial damage.

Xanthelasma and Xanthomas: The Visible Signs of High Cholesterol

While high cholesterol itself is silent, a minority of people develop visible physical signs when cholesterol-carrying particles deposit directly in skin, tendons, or the eye. These are the closest thing to a genuine physical "symptom" of high cholesterol — though they're signs you or a clinician can see, not something you feel.

Xanthelasma

Xanthelasma palpebrarum are soft, yellowish, slightly raised patches that form on or around the eyelids, most often at the inner corner near the nose. According to NIH's clinical review of xanthelasma, they form when LDL cholesterol and other lipids accumulate inside immune cells (macrophages) that then deposit as visible fatty plaques in the thin skin around the eyes. Roughly half of people with xanthelasma have measurably abnormal cholesterol or lipid levels — the other half have normal labs, so the patches themselves aren't a diagnosis. Xanthelasma is painless and doesn't affect vision, but when it appears in someone under about 40-45, it's a specific reason to check for familial hypercholesterolemia (FH), an inherited condition that causes very high LDL from birth and affects an estimated 1 in 250 people.

Xanthomas

Xanthomas are the broader category of the same kind of deposit, appearing anywhere on the body rather than just the eyelids — over tendons (especially the Achilles tendon and finger knuckles), in skin creases, or on elbows and knees. Tendon (tendinous) xanthomas in particular are a classic clinical sign of familial hypercholesterolemia and, like xanthelasma at a young age, warrant a full lipid panel and a conversation with your doctor about inherited lipid disorders.

Corneal arcus

A third, less commonly recognized sign is corneal arcus (sometimes called arcus senilis) — a grayish-white ring that forms around the edge of the cornea. In people over roughly 60, this ring is a normal age-related finding unrelated to cholesterol. But when it appears before about age 45, it's considered a potential marker of familial hypercholesterolemia and, like the deposits above, is a reason to get tested rather than a diagnosis on its own.

None of these three signs are common — most people, even those with meaningfully elevated cholesterol, never develop any of them. Symptoms That Appear Only After Cholesterol Damages Your Arteries

The symptoms most people actually associate with "high cholesterol" — chest pain, leg pain when walking, sudden weakness or slurred speech — aren't caused by the number on a lab report. They're caused by atherosclerosis, the plaque buildup that years of elevated cholesterol can drive inside artery walls, once that plaque has narrowed a specific artery enough to restrict blood flow. You can have high cholesterol for years with zero symptoms while atherosclerosis is quietly progressing; these symptoms show up later, if elevated cholesterol has already caused significant vascular damage — not reliably when a lipid panel first comes back high.

  • Chest pain or pressure (angina) — discomfort, tightness, or pressure in the chest, often triggered by exertion or stress and relieved by rest, is a sign that a coronary artery has narrowed enough to limit blood flow to the heart muscle. Per the American Heart Association, this and related warning signs (discomfort in the arms, back, neck, or jaw; shortness of breath; nausea) can signal a heart attack in progress and warrant calling 911.
  • Leg pain when walking (claudication) — cramping, aching, or fatigue in the calf, thigh, or hip that comes on with walking and eases with rest is the hallmark symptom of peripheral artery disease (PAD), caused by the same atherosclerotic plaque narrowing arteries in the legs instead of the heart.
  • Stroke or TIA warning signs — sudden face drooping, arm weakness, or slurred speech (remembered by the acronym FAST: Face, Arm, Speech, Time to call 911) can signal a stroke, or a transient ischemic attack (TIA) — a "mini-stroke" with the same symptoms that resolve within minutes but carry real future stroke risk.

If you experience any of these — especially sudden new chest pain, one-sided weakness, or slurred speech — call 911 immediately. These are medical emergencies regardless of what your last cholesterol test showed.

Cholesterol Myths: What Usually Isn't Caused by High Cholesterol Alone

Because high cholesterol is so common and so often invisible, it gets blamed for a lot of everyday symptoms it doesn't reliably cause. Being clear about this matters, since chasing the wrong symptom can delay both an accurate diagnosis and an actual lipid panel.

  • Headaches — per Cleveland Clinic, there's no reliable evidence that high cholesterol directly causes headaches. One study found people with migraine with aura tended to have higher cholesterol and triglycerides than people without headaches, but that's an association, not proof that cholesterol caused the headache — and it doesn't apply to everyday tension or non-migraine headaches at all.
  • Fatigue — high cholesterol has no established direct mechanism for causing tiredness. When fatigue and elevated cholesterol occur together, the more likely explanations are shared risk factors (poor sleep, low activity, a high-sugar diet) or, once treatment starts, muscle aches and fatigue as a statin side effect — which is a medication effect, not a cholesterol symptom.
  • Dizziness — like headaches and fatigue, dizziness isn't a recognized direct symptom of elevated cholesterol. If dizziness is frequent, it's worth evaluating separately (blood pressure, inner-ear causes, blood sugar) rather than attributing it to a lipid panel.

The honest takeaway: if you feel unwell, get evaluated for what you actually feel — don't assume it's "probably my cholesterol," and don't assume normal-feeling means normal cholesterol either. The two are largely independent until cholesterol has caused the specific downstream damage described above.

When to Get Tested (Since Symptoms Aren't Reliable)
