Exogenous Ketone Supplements: What the Evidence Shows

Exogenous ketones are supplements, usually beta-hydroxybutyrate (BHB) bound to a salt or an ester, that raise blood ketone levels within an hour, without requiring carbohydrate restriction. The evidence is mixed: one well-controlled study found a ketone ester drink reduced hunger for several hours, ketone esters have shown a small cycling-performance benefit in trained athletes, and ketone salts have shown no benefit or even impaired high-intensity performance. Weight-loss evidence is thin, and both forms carry real downsides: cost, taste, and, for salts, a meaningful mineral load.
Exogenous ketones raise blood ketone levels directly, without the carbohydrate restriction needed to produce nutritional ketosis on a low-carb or ketogenic diet. The mistake we see readers make most often is assuming that this supplement-driven spike is the same as diet-driven ketosis. Exogenous ketone supplements deliver beta-hydroxybutyrate (BHB) in a form you swallow, rather than one your own metabolism generates. BHB is the main ketone body your liver produces during fasting or carbohydrate restriction. Because BHB itself doesn't absorb well on its own, manufacturers bind it to something else to get it into the bloodstream. That binding method sets the two main product types on the market apart from each other.
Ketone salts bind BHB to a mineral such as sodium, potassium, calcium, or magnesium. Ketone esters bind BHB to a chemical group that the body cleaves apart during digestion, releasing free BHB without the mineral attached. Esters raise blood ketones higher and faster than salts. They also cost substantially more, and have a reputation for an unpleasant, bitter taste that limits how much people actually drink.

Neither form is the better choice across the board. Salts suit someone who mainly wants a cheaper, easier-to-drink way to test whether elevated ketones change how they feel. Esters suit someone specifically chasing the performance research, who is willing to pay more and tolerate the taste. Anyone watching sodium intake for blood pressure or kidney reasons should weigh the salts' mineral load, alongside the ketone content on the label, before choosing.
Do They Suppress Appetite?
The appetite evidence is the most encouraging part of the research. It still comes from a single well-designed study rather than a broad literature. A randomized trial by Stubbs and colleagues, published in Obesity in 2018, gave 15 participants a ketone ester drink after an overnight fast. Hunger ratings dropped. So did the hunger hormone ghrelin, and both stayed lower for up to four hours compared with a calorie-matched placebo drink.
One trial is not the whole picture. This result came from a single small study, in participants drinking on an empty stomach. It also used a ketone ester rather than the cheaper salt form most consumer products actually sell. Whether the effect holds after a normal meal, over weeks rather than hours, or with a salt instead of an ester is not yet well established.
Do They Help with Weight Loss?
The direct weight-loss evidence is thin, and what exists doesn't clearly support the claim. Exogenous ketones carry their own calories, so simply adding them to an unchanged diet adds energy intake rather than removing it. Some researchers have raised the opposite concern: a rise in circulating ketones could blunt the body's own fat-burning response. One signal that normally drives fat breakdown is a low, not elevated, insulin-to-ketone ratio.
The appetite-suppression finding above is the closest thing to an indirect weight-loss mechanism the research supports, and even that rests on one small trial. The evidence doesn't yet support taking exogenous ketones as a stand-alone weight-loss strategy, separate from the diet and activity changes that drive fat loss.
Do They Improve Athletic Performance?
Performance results split sharply by product type. A 2016 trial by Cox and colleagues, published in Cell Metabolism, tested a ketone ester drink against a calorie-matched carbohydrate drink in trained cyclists. The ester group improved a 30-minute cycling time trial by about 2%, by shifting which fuel the muscles preferred to burn.
Ketone salts have not replicated that result. A 2017 trial by O'Malley and colleagues, published in Applied Physiology, Nutrition, and Metabolism, found that a ketone salt supplement increased fat oxidation during exercise. But it also left time-trial power output about 7% lower than the control drink, likely because the added mineral load and GI discomfort outweighed any fuel-shifting benefit.
The one positive performance result on record used an expensive ketone ester, in trained athletes, doing a specific endurance protocol. It didn't use the cheaper salt products most gyms sell, and it hasn't been consistently reproduced across other exercise types.
Side Effects and Practical Downsides
Gastrointestinal upset, including nausea, bloating, and diarrhea, is the most commonly reported side effect with both forms. It tends to get worse at the higher doses needed to meaningfully raise blood ketones. Ketone salts add a second issue: the mineral dose. A full serving can carry close to a gram of sodium or more. That adds up quickly for anyone also managing blood pressure or kidney health, and stacking multiple daily doses compounds the mineral load further.
Cost is a real practical barrier on both forms. Esters in particular run high enough per serving that sustained daily use gets expensive fast, relative to the modest, short-lived effects the research supports. The bitter taste of esters is enough on its own to make consistent use difficult for many people. That matters because most of the studied benefits were measured after a single full dose, not a partial one taken to make the taste tolerable.
Who Should Be Cautious
Anyone with kidney disease or a condition requiring sodium or potassium restriction should talk to a doctor before using ketone salts specifically, given the mineral load involved. There is no meaningful safety data on exogenous ketones during pregnancy or breastfeeding, so avoiding them in that situation is the cautious default. Anyone with diabetes, especially someone taking an SGLT2 inhibitor, should also take note. A supplement-driven rise in blood ketones can complicate reading a home ketone meter, making it harder to tell a normal supplement effect from an early warning sign of ketoacidosis. Do exogenous ketones put you in ketosis?
They raise blood ketone levels directly, which can read the same as nutritional ketosis on a home meter, but the underlying metabolic state differs. Diet-driven nutritional ketosis reflects a body that has adapted to burning fat for fuel. A supplement-driven ketone spike is temporary and doesn't require that metabolic shift, so it fades within a few hours regardless of what you eat.
Can you take exogenous ketones without a low-carb diet?
Yes, and most of the research trials did exactly that, testing the supplement on its own rather than layered on a ketogenic diet. Taking exogenous ketones on a higher-carb diet raises blood ketones temporarily but doesn't produce the sustained metabolic adaptations, such as improved insulin sensitivity, associated with an actual low-carb eating pattern.
Are ketone salts or ketone esters better?
Neither is better across every use case. Ketone esters raise blood ketones higher and have the one positive performance trial behind them, but cost more and taste considerably worse. Ketone salts are cheaper and easier to drink, but carry a meaningful mineral load and haven't shown a performance benefit in trials.
