Low Vitamin D Linked to Higher Pain After Breast Cancer Surgery

A prospective study finds patients with deficient levels required substantially more tramadol and reported three times the rate of moderate pain.

Low Vitamin D Levels Tripled Moderate Postoperative Pain

Patients with vitamin D deficiency faced three times the rate of moderate postoperative pain during the first 24 hours after a radical mastectomy. That finding comes from a prospective observational study published in Regional Anesthesia and Pain Medicine by researchers at Fayoum University Hospital in Egypt. The team followed 184 women undergoing surgery between September 2024 and April 2025 to evaluate how baseline blood levels altered recovery.

Clinical staff divided participants into two equal groups of 92 based on blood concentrations. One cohort had vitamin D levels below 30 nanomoles per liter (nmol/L), which researchers defined as deficient. The other group had levels above 30 nmol/L, meeting the threshold for sufficiency. The deficient cohort had an average age of 44, while the sufficient group averaged 42 years of age.

Pain tracking revealed a focused difference during the initial recovery day. Neither group reported severe pain, defined as a score of 7 or higher on a 10-point scale. The entire increase in pain reports came from moderate scores between 4 and 6, recorded across scheduled checks at zero, 6, 12, 18, and 24 hours.

Vitamin D Deficiency Increased Tramadol and Fentanyl Use

Deficient vitamin D levels corresponded with higher surgical and postoperative opioid requirements. During surgery, patients with low levels received an average of 8 micrograms more fentanyl than those with sufficient levels. Researchers classified that intraoperative difference as modest, but a larger separation occurred once patients woke up.

All 184 participants received standard intravenous paracetamol every 8 hours after surgery. For additional relief, patients operated a patient-controlled system providing tramadol up to a limit of 50 milligrams per hour. Patients in the vitamin D-deficient group used 112 milligrams of tramadol, which the authors characterized as substantially more medication than sufficient peers required.

Higher opioid intake brought measurable recovery burdens. Patients with deficient levels experienced higher rates of postoperative nausea. Opioid medications carry known clinical tradeoffs, including nausea, vomiting, sedation, drowsiness, confusion, and risks of dependency.

How Vitamin D May Influence Inflammation and Pain Signals

Vitamin D interacts directly with immune responses and systemic inflammation pathways that process pain. While public attention centers on skeletal integrity and bone density, nerve pathways rely on sufficient vitamin D activity to regulate sensory thresholds. Low circulating concentrations may lower pain thresholds or amplify inflammatory signaling following major surgical incisions.

Breast cancer patients frequently present with baseline vitamin D deficiency before treatment begins. Radical mastectomy involves removing an entire breast, creating substantial tissue trauma. When tissue injury pairs with low vitamin D, altered immune regulation may leave nerves more reactive to early physical stress.

Observational Design Leaves Open Questions on Supplementation

Observational research cannot prove that taking vitamin D supplements before surgery will reduce pain. The investigators tracked clinical outcomes forward in time without assigning patients to active supplementation or placebo treatments. This means the data shows an association between low levels and higher pain, rather than proving low vitamin D directly caused the pain.

The treating physicians and nurses did not know the vitamin D measurements of the patients in their care. This double-blind design feature prevented clinicians from altering standard care based on lab numbers. Even with that precaution, randomized clinical trials are required to test whether correcting a deficiency before a mastectomy actually reduces opioid needs.

Future trials must establish specific target levels, appropriate intervention windows, and clear dosing protocols before surgery. Until interventional trials publish findings, clinicians cannot assume that rapid preoperative supplementation provides immediate pain protection.

Testing vitamin D levels before planned surgical procedures gives patients and physicians objective baseline information. A simple 25-hydroxyvitamin D blood test identifies whether levels sit below the 30 nmol/L mark evaluated in this research.
