Thymalin's Longevity Claims Rest on One Unblinded Trial

The mortality result everyone quotes came from an unblinded 266-person observation.

Russia registers Thymalin as a medicine. The United States has never approved it and sells it only as a research chemical. Mortality roughly halved in the study people quote, though the peptide was given only in the first two to three years of a six to eight year follow-up. A 2021 paper reported faster recovery from lymphopenia in 36 hospitalized COVID-19 patients.

Thymalin is an extract of calf thymus. At The Metabolic Journal we read the trial behind a longevity peptide before the marketing written about it. Thymalin has one trial doing almost all of the work.

That study describes no randomization and no blinding. Vendor pages selling the vial quote the mortality figure and leave the design out.

Thymalin also gets confused with two other thymus compounds that are chemically different from it. If you came here after seeing thymulin or thymosin alpha-1 used as if they were the same product, the table below separates them.

What Thymalin Is Made Of

Thymalin is a purified polypeptide mixture pulled from bovine calf thymus tissue, so it has no single amino acid sequence to quote. Most peptides sold alongside it are one defined molecule made to a spec. A batch of Thymalin is a preparation, and two batches are only as alike as the extraction that produced them.

Khavinson's group at the St. Petersburg Institute of Bioregulation and Gerontology has since identified short peptides inside the mixture. The 2021 paper in Advances in Gerontology names KE, EW and EDP as the fragments it credits for the effect. It proposes that they bind deoxyribonucleic acid (DNA) and histone proteins to change gene expression. That mechanism claim is why sellers list Thymalin under the "epigenetic peptide" label.

For a reader, the practical consequence is that purity testing means less here than it does elsewhere. A certificate on a single-sequence peptide confirms the molecule matches the label. On a tissue extract there is no single molecule to match, so the number tells you less about what is in the vial. The 266-Person Study Behind the Thymalin Longevity Numbers

Every mortality figure quoted for Thymalin traces back to one paper: Khavinson and Morozov, published in Neuroendocrinology Letters in 2003. Researchers at the St. Petersburg Institute of Bioregulation and Gerontology and the Institute of Gerontology in Kiev followed 266 elderly people for six to eight years. You can read the paper itself.

The reported results are large. Mortality fell 2.0 to 2.1-fold in the group given Thymalin, 1.6 to 1.8-fold with Epithalamin, and 2.5-fold with both. Acute respiratory disease incidence fell 2.0 to 2.4-fold. A separate group given both peptides annually for six years showed a 4.1-fold drop. That last number gets lifted onto product pages without its condition attached.

Two details in the methods change how much weight the numbers carry. The paper never describes randomizing patients or blinding anyone, which leaves selection and expectation effects unaccounted for. And the peptides were given only during the first two to three years of a six to eight year observation, so most of the follow-up happened after dosing stopped.

None of that makes the result wrong. It makes it a strong signal from a weak design, which is a normal place for a compound to sit before anyone replicates it. Twenty-three years later, no Western group has run the replication.

What the COVID-19 Trial Added

The most recent clinical work on Thymalin is a 2021 COVID-19 study in older patients with severe disease. Thirty-six patients received Thymalin alongside standard therapy. They improved faster, a higher proportion recovered from lymphopenia, and their C-reactive protein normalized sooner than in the comparison group.

The dose used was 10 mg given intramuscularly each day for ten days. That is the figure the study reports, in hospitalized patients under supervision, for a specific acute illness. It is not a protocol for taking a research chemical at home, and nothing in the paper supports extending it to healthy adults.

The literature as a whole has the same shape. A PubMed search for thymalin returns a few hundred records dominated by Russian-language work from the same institutes, much of it in animals. Volume of publication is not the same as independent replication.

Thymalin Is Not Thymulin or Thymosin Alpha-1

Three different thymus compounds circulate under names close enough to swap by accident, and they are not interchangeable. Buying the wrong one wastes money and produces a result no study predicts. The differences are structural.

VerdictThymulin has the clearest mechanism. Thymosin alpha-1 has regulatory approval outside the United States. Thymalin has the longevity numbers and the weakest study design.

Thymulin is the compound most often mistaken for Thymalin. It has a published sequence, and its biological activity depends on binding zinc in equal measure, which a review of zinc and thymulin lays out. Thymalin has no sequence to depend on anything. A vendor listing that treats the two as the same product has not read either literature.

The FDA Has Never Approved Thymalin

Thymalin is a registered medicine in Russia and has no FDA approval in the United States. Everything sold here moves through research-chemical channels, labeled not for human use. It is not a controlled substance, so holding a vial breaks no law. The risk sits in an unregulated supply chain, where nobody verifies what a vial contains.

The FDA has named Thymalin directly. Its February 2024 warning letter to US Chem Labs cited Thymalin as an unapproved new drug and as misbranded. The letter quotes the seller's own claim about treating "immunodeficiency in adults and children (from 6 months to 14 years)". The FDA singled out that age range.

No compounding route exists either. Thymalin was not among the seven peptides the FDA's Pharmacy Compounding Advisory Committee reviewed in July 2026, so no pharmacy has a lawful path to compound it. The peptide that committee turned down was emideltide, a sleep and opioid-withdrawal compound with no relation to Thymalin, which our page on the emideltide vote covers. Who Should Skip Thymalin

Anyone taking an immunosuppressant should not add Thymalin. Transplant recipients, people on biologics for an autoimmune condition, and anyone on a treatment that damps immune activity are working against a compound sold for immune stimulation. Take that question to the clinician managing the prescription before anything else.

Skip it if you are pregnant, nursing, or under 18. No study has looked at any of those groups, and the FDA letter above exists because a seller marketed it to children anyway.

The budget case is also weak for a first peptide. Thymalin is an animal extract with one unblinded human study, which puts it behind compounds that have defined sequences and replicated trials. If immune support is the goal and you want the better-documented option, thymosin alpha-1 has more regulatory history. If longevity is the goal, spend the money on the supplements with human outcome data first.

What Would Change This Answer

One randomized, blinded replication would change it. The 2003 mortality result is large enough that a properly designed trial in 300 older adults would settle the question in either direction within a few years. No group outside the original institutes has attempted it.

A standardized preparation would also change it. If Thymalin were made to a published composition, with each batch assayed for the KE, EW and EDP content the 2021 paper credits, buyers could verify what they hold. Right now a certificate on a tissue extract cannot tell you that.

Until one of those lands, treat Thymalin as an unreplicated result rather than a protocol. If you are still considering it, bring the 2003 paper to a clinician who manages peptide therapy. Ask them to read its methods section with you before you buy a vial of Thymalin.

Frequently Asked Questions

Is Thymalin safe?

No study large enough to characterize its safety has been run. The published human work reports few adverse events, but it covers a few hundred people total, mostly under medical supervision in Russia. Anyone on an immunosuppressant, pregnant, nursing, or under 18 should not take it.

Is Thymalin the same as thymulin?
