Most of this site's biomarker coverage focuses on metabolic and hormonal health — insulin, cortisol, testosterone, and similar markers people track proactively. But the word "biomarker" is used much more broadly in medicine, including for tests that screen for or monitor serious disease: cancer, heart disease, kidney and liver dysfunction, and neurodegenerative conditions like Alzheimer's. This page is a brief, general orientation to that broader category — what these markers are and why they're ordered — not a substitute for the specialty-specific care these conditions require.

What Makes a Biomarker "Disease-Specific"

In this context, a biomarker is any measurable substance — usually in blood, but sometimes urine, tissue, or imaging findings — whose level or presence correlates with a specific disease process. Disease-screening biomarkers generally fall into a few categories: tumor markers used alongside imaging and biopsy in certain cancers (like PSA for prostate cancer, which we cover separately at PSA levels); cardiac biomarkers like troponin and BNP that rise during a heart attack or heart failure; markers of organ function (elevated liver enzymes, declining kidney eGFR); and emerging markers under active research for neurodegenerative disease, such as specific proteins studied in Alzheimer's disease diagnosis.

Almost none of these are used as a standalone diagnosis. A cancer biomarker that's elevated doesn't mean a person has cancer, and a normal result doesn't rule it out — these tests are one input alongside imaging, biopsy, symptoms, and clinical judgment. That's a meaningfully different use case than the proactive wellness tracking most of this site discusses, where a marker like fasting insulin is genuinely actionable and trackable on its own over time.

Cardiac Biomarkers

The best-known example is troponin, a protein released into the bloodstream when heart muscle is damaged. It's the primary blood test used in emergency departments to confirm or rule out a heart attack, typically drawn serially over several hours because levels rise progressively after cardiac injury. BNP (B-type natriuretic peptide) is another cardiac biomarker, released when the heart is under strain, and is used to help diagnose and monitor heart failure. Neither of these is something to self-order for routine wellness tracking — they're acute-care tools, used when there's clinical suspicion of a cardiac event, not proactive screening markers for an otherwise healthy person. Cancer, Kidney, Liver, and Neurodegenerative Markers

Cancer: Tumor markers (like CA-125, CEA, or PSA) can be elevated in certain cancers but are also elevated by many benign conditions, which is why they're used to monitor known disease or alongside imaging and biopsy — rarely as a standalone screening tool in an otherwise asymptomatic person, with a few specific exceptions your doctor would guide you on.

Can I get cancer biomarkers tested proactively without symptoms?

Some direct-to-consumer testing services include select tumor markers in broader panels, but most major medical organizations do not recommend routine cancer biomarker screening in asymptomatic, average-risk people outside established guidelines (like age-based mammography or colonoscopy), because these markers can be falsely elevated by benign conditions and cause unnecessary anxiety and follow-up testing. Discuss appropriate cancer screening with your physician based on your personal and family risk factors.

What's the difference between a cardiac biomarker and a cardiovascular risk marker?

Cardiac biomarkers like troponin and BNP detect acute heart damage or strain happening right now — they're used in emergency and hospital settings. Is a normal biomarker result enough to rule out disease?
