How Common Thyroid Nodules Are

Thyroid nodules are lumps that form within the thyroid gland, and they are common: detectable on ultrasound in a large share of adults, especially with age. The American Thyroid Association estimates that only about 5% of thyroid nodules turn out to be cancerous. Ultrasound features determine whether a nodule needs a fine-needle aspiration biopsy, following the American College of Radiology's TI-RADS risk-stratification system.

A thyroid nodule is a lump of cells that grows within the thyroid gland, forming a distinct area that feels or looks different from the surrounding tissue. The American Thyroid Association (ATA) reports that nodules are detectable in a substantial share of the adult population when a sensitive enough scan is used, and the rate climbs with age.

Most people never feel a thyroid nodule or notice any symptom from it. Many are found incidentally on an ultrasound or CT scan ordered for an unrelated reason. Thyroid Nodule vs. Goiter: What's the Difference?

A thyroid nodule is a single, distinct lump within the gland. A goiter is a general enlargement of the entire thyroid. A person can have either one without the other. A multinodular goiter describes a third pattern: an enlarged thyroid that also contains several distinct nodules rather than one. That distinction changes the imaging approach, since each nodule in a multinodular goiter gets its own risk assessment rather than one combined score for the whole gland. How Common Are Thyroid Nodules?

Thyroid nodules are common. How common depends heavily on how closely someone looks. Nodules large enough to feel on a physical exam show up in a small share of adults. High-resolution ultrasound, a far more sensitive tool, detects nodules in a much larger share of the population, rising substantially with age. Women are diagnosed with thyroid nodules more often than men, a pattern consistent across the imaging studies the ATA cites in its nodule-management guidelines.

There's a gap between how many nodules ultrasound finds and how many ever cause a symptom. That gap is the single most important context for understanding a new nodule diagnosis. Finding one is common. Finding one is rarely, by itself, a sign that something has gone wrong.

Are Thyroid Nodules Cancerous?

Most thyroid nodules are benign. The ATA puts the share of nodules that turn out to be malignant at roughly 5%. That means the large majority identified on a scan require monitoring rather than removal. That 5% figure is why professional guidelines focus on risk-stratifying nodules by their ultrasound appearance instead of recommending biopsy for every nodule found.

Certain features raise suspicion enough to warrant a closer look:

  • Solid composition rather than fluid-filled
  • Irregular or poorly defined margins
  • A markedly darker appearance than surrounding tissue on ultrasound (hypoechoic)
  • Tiny calcium deposits called microcalcifications
  • A shape taller than it is wide

No single feature alone confirms cancer. Radiologists combine them into a composite risk score before recommending next steps.

What Symptoms Do Thyroid Nodules Cause?

Most thyroid nodules cause no symptoms at all and are found incidentally during an unrelated exam or scan. When a nodule does cause symptoms, the most common are:

  • A visible or feelable lump in the front of the neck
  • A sense of fullness or pressure in the throat
  • Difficulty swallowing, if the nodule is large enough to press on the esophagus
  • Hoarseness, less commonly, if it presses on a nerve near the voice box

A minority of nodules, called "hot" or hyperfunctioning nodules, produce excess thyroid hormone on their own, independent of the pituitary signal that normally controls thyroid output. These can cause hyperthyroidism symptoms such as a racing heart, weight loss, or anxiety. A blood test showing a suppressed TSH alongside a nodule is often the first clue that points a clinician toward a hot nodule rather than a purely structural one. How Are Thyroid Nodules Diagnosed?

Ultrasound is the first and most informative imaging test for a thyroid nodule, whether it was found on a physical exam or incidentally on another scan. Radiologists use the American College of Radiology's TI-RADS (Thyroid Imaging Reporting and Data System) to score a nodule's ultrasound features and translate that score into a specific recommendation. Some low-risk nodules need no biopsy at all. Others need biopsy only above a certain size threshold. Higher-risk features lower the size threshold at which biopsy gets recommended.

A blood test measuring TSH (thyroid-stimulating hormone) typically accompanies the ultrasound. A nodule found alongside abnormal TSH points toward a hyperfunctioning nodule rather than a purely structural finding, which changes the workup. When Does a Thyroid Nodule Need a Biopsy?

A fine-needle aspiration (FNA) biopsy gets recommended when a nodule's TI-RADS risk category and size together cross the threshold that category's guidelines set. It isn't done for every nodule an ultrasound finds. The procedure uses a thin needle, guided by real-time ultrasound, to withdraw a small sample of cells from the nodule for a pathologist to examine. It's typically an outpatient procedure, done without general anesthesia.

A biopsy result usually falls into one of a small number of standardized categories, ranging from clearly benign to clearly malignant. A few intermediate categories sometimes call for a repeat biopsy or molecular testing on the sample rather than an immediate answer. A benign result typically leads to periodic follow-up ultrasound rather than further intervention, at an interval the ordering clinician sets based on the nodule's original risk category.

Can Thyroid Nodules Shrink or Go Away on Their Own?

Some thyroid nodules can shrink over time or after a fluid-draining procedure, particularly ones that are largely fluid-filled (cystic) rather than solid, though they can also recur. Solid nodules are far less likely to shrink without treatment. These are typically managed with monitoring rather than an expectation that they will resolve unassisted. Whether a specific nodule is likely to change over time depends on its composition and size. That's a question for the clinician who read the original ultrasound. A general guide cannot predict it for an individual case.

Can thyroid nodules cause weight gain?

Only indirectly, and only in a minority of cases. Most thyroid nodules do not affect hormone levels at all and would not be expected to cause weight change on their own.

How often should thyroid nodules be checked?

It depends on the nodule's original TI-RADS risk category and whether it was biopsied. The clinician who read the original ultrasound sets the specific schedule, typically ranging from about six months to a few years between checks for a benign or low-risk finding. There is no single interval that applies to every nodule.

Are thyroid nodules hereditary?

Family history plays a role, but most cases occur without one. A family history of thyroid nodules or thyroid cancer raises the likelihood of developing a nodule and is one factor clinicians weigh when assessing risk, though most nodules occur in people with no known family history. Certain rare inherited thyroid cancer syndromes exist and are typically identified through a genetics workup rather than by family history alone.

Does thyroid nodule size determine if it's cancerous?

No, size by itself does not decide it. Size affects the biopsy threshold within a given TI-RADS risk category, but the category itself comes from the ultrasound's other features. A small nodule with high-risk features can be biopsied sooner than a larger nodule with reassuring ones. The composite ultrasound risk score drives the recommendation ahead of size alone.
