Review Examines Metformin’s Potential Role in Slowing Aging
A new review compiles decades of research suggesting metformin may target biological aging mechanisms, but evidence in humans remains mixed.
Metformin’s Traditional Role and New Interest in Aging
Metformin is a widely prescribed, low-cost drug for type 2 diabetes that lowers blood sugar and improves insulin response. It works by activating a cellular energy sensor called AMPK, which helps trigger cellular cleanup and repair processes.
A recent review, published in Aging, suggests metformin may also counter some effects of aging by targeting biological root causes, potentially improving health span beyond its use in diabetes.
What the Evidence Shows: From Cells to Humans
The review compiles decades of research from cellular, animal, and human studies to provide a unified picture of metformin’s effects on aging. In animal studies, male monkeys treated with metformin for 40 months showed tissues that became biologically younger, with brain aging reversed by nearly 6 years—an effect equivalent to 18 years in human terms.
In humans, observational data indicate that people taking metformin had biological ages up to 3.43 years younger, and people with diabetes taking the drug lived as much as 15% longer than peers without diabetes. However, most human studies have focused on people with diabetes, making it difficult to separate metformin’s effects on aging from its effects on the underlying disease.
How Metformin May Affect Aging and Metabolic Health
Metformin appears to act as a geroprotective agent, targeting multiple biological mechanisms of aging at once. It can trigger autophagy, the cell’s waste removal system, and support DNA stability by affecting methylation patterns that change with age.
The drug may also influence the gut microbiota and has been linked to fewer cardiovascular events and lower rates of cognitive decline in people prescribed metformin for blood sugar management.
Limitations and Ongoing Questions
Despite promising findings, the evidence for metformin’s anti-aging effects in humans is inconsistent. A widely cited study linking metformin to longer survival in humans has faced criticism after other researchers could not reproduce its results.
Most human data come from people with diabetes, so it is unclear whether metformin’s potential anti-aging benefits extend to people without diabetes. The review notes that findings from cell, animal, and human studies are mixed, and more research is needed to clarify metformin’s role in aging.
Practical Context for Metabolic Health
People interested in metabolic health may be curious about metformin’s possible benefits beyond blood sugar control. The review highlights that metformin is being studied as a drug that could target aging itself, rather than treating age-related diseases one by one.
However, the current evidence does not support using metformin specifically for aging in people without diabetes, and most findings are based on observational data or studies in animals.
