New Weight-Loss Injection CBL-514 Reduces Abdominal Fat
Phase 2 trial data presented at EASD 2026 shows targeted adipocyte destruction and reductions in visceral fat.

A new weight-loss injection CBL-514 reduced visceral fat volume in human participants during early clinical testing. Researchers presented the subgroup findings at the annual meeting of the European Association for the Study of Diabetes (EASD) in Milan, Italy. The drug is an investigational compound developed by Caliway Biopharmaceuticals in New Taipei City, Taiwan.
CBL-514 functions by selectively inducing apoptosis, or programmed cell clearance, inside adipocytes while leaving adjacent tissue undamaged. Data collected from human trials CBL-0204 and CBL-0205 evaluated the physical changes in abdominal fat after localized administration. Lead investigator Dr. W. Timothy Garvey of the University of Alabama at Birmingham reported that treated participants lost both subcutaneous and deeper abdominal fat.
Human Data on Visceral and Subcutaneous Fat Loss
Magnetic resonance imaging (MRI) scans tracked changes in visceral adipose tissue (VAT) among 40 adult participants. The cohort included individuals with a body mass index (BMI) between 22 and 30 kg/m2 and an average age spanning 36.4 to 42.4 years. Participants received abdominal injections of up to 600 mg of CBL-514 (n=23) or a placebo (n=17) every three weeks for up to 12 weeks.
Average VAT volume dropped by 12.01% from baseline in the CBL-514 group four weeks after the final injection. Placebo recipients experienced a 5.68% increase over the same window, creating a 17.69 percentage point difference between the groups. By week eight to week 12 post-treatment, VAT remained 10.45% below baseline in treated participants compared to an 11.76% increase in the placebo group.
Subcutaneous tissue showed parallel reductions during the Phase 2 program. In earlier trial assessments, 69.6% of participants receiving CBL-514 lost at least 150 mL of subcutaneous fat at the eight-week follow-up, whereas 0% of placebo recipients met that mark.
Animal Studies and Weight Regain After GLP-1 Therapy
Preclinical rodent models suggest the new weight-loss injection CBL-514 may help limit weight regain after stopping other obesity therapies. When researchers administered CBL-514 alongside the glucagon-like peptide-1 receptor agonist (GLP-1 RA) tirzepatide, the animals lost more total weight than those on single-agent therapy. After stopping tirzepatide, rats previously treated with both compounds regained less body mass than rats given tirzepatide alone.
Investigators hypothesize that clearing fat cells permanently limits the biological storage space available for rapid lipid reaccumulation. Many patients regain lost mass after stopping GLP-1 RA medications. If confirmed in humans, preventing rapid fat regain could help preserve improvements in resting blood pressure, blood glucose regulation, and lipid panels.
Safety Profile and Trial Limitations
Reported adverse events in the Phase 2 subgroup were primarily localized to the injection site. Participants experienced transient pain, swelling, and redness that investigators graded as mild to moderate. The study recorded no serious drug-related adverse events across the monitored period.
The findings carry important clinical limitations. The human VAT analysis relies on a small sample size of 40 participants across two trials. Evidence regarding post-treatment weight maintenance currently exists only in rodent models. Human clinical trials must verify whether the new weight-loss injection CBL-514 prevents weight regain in patients stopping GLP-1 RA regimens.
