Ketone Ester Clinical Trial Tests Health Span in Older Adults

Researchers at University of Connecticut Health (UConn Health) are running a randomized clinical trial to test whether a ketone ester supplement improves physical and cognitive function in older adults. Jenna Bartley, Ph.D., leads the multisite investigation to evaluate whether raising circulating ketones can extend health span in individuals showing early signs of functional decline.
Earlier clinical studies established the safety and feasibility of ketone ester supplementation in older cohorts. The current trial evaluates whether regular intake can alter physiological markers associated with physical decline and cognitive slowing.
Ketone Signaling and Metabolic Function
Ketones serve as signaling molecules within human tissue in addition to acting as an alternative fuel source when carbohydrate availability drops. The body naturally synthesizes ketones during fasting or significant carbohydrate restriction when fat metabolism replaces glucose as the primary energy pathway.
Emerging laboratory research indicates that circulating ketones can influence muscle metabolism, metabolic regulation, and immune function. The trial examines whether oral supplementation can deliver those signaling effects without requiring participants to follow a restrictive ketogenic diet. This geroscience strategy targets biological aging processes directly rather than addressing a single isolated disease.
Protocol for Pre-Frail Study Participants
The trial enrolls adults aged 65 and older who meet clinical criteria for pre-frailty. The pre-frail classification identifies older adults who are beginning to experience functional slowing but have not yet developed advanced physical frailty.
Enrolled participants complete a 20-week protocol in which they take four packets of a study product daily, divided into two doses. Volunteers receive either the active ketone ester supplement or an inactive placebo under blinded conditions. The research design requires participants to maintain their normal diet and daily routines without adding new exercise programs. Participants wear provided activity trackers, undergo blood testing, and complete biweekly check-ins with study staff to monitor symptoms.
Primary Outcomes and Clinical Limitations
The primary outcome of the trial is a composite vigor-frailty score that combines measures of physical capacity and cognitive processing speed. Research staff evaluate leg strength, body composition, and performance on standardized cognitive tasks across four scheduled clinic visits. Secondary endpoints assess changes in immune markers and muscle metabolism.
Clinical data demonstrating that ketone esters improve human health span remain limited. Because the trial remains blinded, individual experiences during the study cannot be linked to the active supplement until final data analysis is complete.
