GLP-1s Linked to Lower Psychiatric Hospitalization in Schizophrenia
An observational study of 70,000 adults found reduced acute psychiatric care risks in patients taking specific diabetes medications.
Lower Risk of Psychiatric Acute Care
Adults with schizophrenia and type 2 diabetes (T2D) who received glucagon-like peptide-1 (GLP-1) receptor agonists faced lower rates of psychiatric hospitalization and emergency department (ED) visits.
Researchers published the observational cohort study in The Lancet Psychiatry after reviewing U.S. electronic health records from January 2018 to October 2026. The dataset included more than 70,000 adults diagnosed with both conditions.
Schizophrenia increases the risk of developing T2D, and antipsychotic medications frequently prescribed for the psychiatric condition elevate that metabolic risk further.
Comparing GLP-1s with Other Diabetes Therapies
Patients on GLP-1 medications experienced fewer acute mental health events than those on other diabetes regimens or no therapy.
Compared to individuals not taking any of these diabetes medications, patients prescribed GLP-1 drugs had a 24 percent lower risk of psychiatric hospital admission. They also showed a 29 percent lower risk of psychiatric ED visits.
GLP-1 treatment also compared favorably against newer diabetes drug classes, such as dipeptidyl peptidase-4 (DPP-4) inhibitors and sodium-glucose cotransporter-2 (SGLT2) inhibitors.
- 18 percent lower admission risk and 13 percent lower ED presentation risk compared to DPP-4 inhibitors.
- 17 percent lower admission risk and 20 percent lower ED presentation risk compared to SGLT2 inhibitors.
Weight Loss and Drug-Specific Differences
The observed reductions in acute psychiatric care were not identical across all medications within the GLP-1 drug class.
When investigators evaluated each drug individually, the association appeared with semaglutide, liraglutide, and tirzepatide, but dulaglutide showed no such association.
The risk reduction was strongest in patients who lost weight during treatment. A smaller reduction remained evident in patients whose weight stayed stable or increased, indicating that metabolic effects beyond weight reduction may play a role.
Study Limitations and Clinical Implications
The study authors emphasized that these observational health records cannot establish that GLP-1 medications directly prevent psychiatric crises.
The researchers stated that clinicians should not treat GLP-1 drugs as primary psychiatric therapies or prescribe them solely to avoid acute mental health care. Anyone reviewing metabolic medications should talk to their clinician about integrated care plans.
The authors concluded that future randomized clinical trials must test these outcomes directly, including in individuals with schizophrenia who do not have T2D.
What to Look For in a Body-Composition Scale
A bioimpedance scale trends body fat and muscle rather than just weight — useful when the number stalls but your composition is improving. The absolute body-fat % isn't lab-accurate, so use it for TRENDS at a consistent time of day. Choose one that syncs to an app so you can watch the trend line.
