What Testing Finds Inside Gray Market Peptide Vials

If you already hold vials, the certificate that came with them does not cover sterility or dose.

The federal record names four failure modes for these products: contamination, counterfeit, wrong amount of active ingredient, and a different ingredient altogether. Each failure mode needs its own assay, and a purity number detects none of them.

Documented failures in gray-market peptide vials include the wrong dose, the wrong salt form, and contamination that nobody screened for. What the Federal Record Says Is in These Vials

The FDA's own description of unapproved and misbranded drugs names four failure modes, and it repeats them in letter after letter. In its February 2025 letter to USApeptide.com the agency puts it this way. Drugs that have circumvented regulatory safeguards may be contaminated, may be counterfeit, may contain varying amounts of active ingredients, or may contain different ingredients altogether.

Read that list as four separate tests rather than one worry. Contamination is caught by sterility and endotoxin assays. Counterfeit and wrong-ingredient products are caught by mass spectrometry identity confirmation. Varying amounts are caught by a quantity measurement. A purity percentage on its own catches none of the four, which is why a vendor certificate showing 99% purity is not the reassurance it looks like.

No vendor certificate covers sterility, and an injection bypasses the defenses that stop a swallowed contaminant. Research-grade powder was never made to an injectable standard in the first place. That is not an accusation about any particular seller. "Research use only" describes exactly that manufacturing spec.

Wrong Salt Forms Are a Different Molecule

Salt form substitution is a documented problem, and a buyer cannot see it. When the FDA launched its green list on 5 September 2025, it gave three reasons for acting. Unapproved salt forms was one, alongside dosing errors and hospitalizations.

A salt form is the counterion the peptide is paired with, and swapping it changes the substance. Semaglutide sodium and semaglutide acetate are not the semaglutide that was studied and approved, and no safety or effectiveness data carries across from one to the other. A label saying "semaglutide" does not commit a supplier to which one is in the vial.

The Dosing Error Is the Documented Harm

The clearest documented harm in this category is a measurement error. The FDA has published a specific alert on dosing errors with compounded injectable semaglutide, reporting adverse events that in some cases required hospitalization.

Many patients receiving vials had never self-injected, were unfamiliar with drawing medication from a vial into a syringe, and confused milliliters with milligrams and with syringe "units". Concentrations also differ between products, so the same volume is a different dose depending on whose vial it came from.

A case series reported to a regional poison control center records three adverse drug events after incorrect semaglutide administration, and two were ten-fold dosing errors. All three brought nausea, vomiting and abdominal pain lasting days. Those patients had sourced the drug from compounding pharmacies and an aesthetic spa rather than a research-chemical site. Semaglutide has a half-life of about a week, so an overdose is not something that clears by morning.

What to Do If You Already Have Vials

Work through this in order. The first two steps cost nothing and resolve most cases.

  1. Stop before the next dose. Nothing below changes if you wait, and an unknown vial is no more knowable after another injection.
  2. Find the lot number on the vial and the certificate the seller supplied. If the lot numbers do not match, the certificate describes different powder and you can stop evaluating it.
  3. Try to verify the certificate at the issuing laboratory. Freedom Diagnostics publishes a lookup that takes an accession number or a company name, and Janoshik reports can be checked against the code printed on them. A certificate that cannot be checked at the lab that supposedly wrote it is an image file.
  4. Search the seller in the FDA warning letter database. Letters are published in full and name the products cited.
  5. Decide between testing and discarding. Testing one vial runs roughly $200 to $250 at a US lab, or free through Finnrick for US residents in exchange for publishing the result. Our comparison of testing labs covers who runs what.
  6. Tell a clinician what you took, if you took any. Embarrassment stops people making this call. Make it anyway if symptoms appear.

How to Read the Result You Get Back

A test result answers a narrower question than most people expect, so decide in advance what each outcome will change.

A clean identity and quantity result tells you the vial you sent held roughly what the label claimed, on the day it was tested. It does not clear the rest of the batch, because the lab tested one vial. It does not address sterility unless you paid for that assay separately.

A failed result is the more useful outcome, because it is actionable. Wrong identity, a salt form you did not order, or a quantity well under the label all mean the same thing: discard the batch and stop buying from that source. Do not try to correct for an underdose by drawing more, which is the exact error the FDA alert describes.

Who Should Skip Testing and Simply Stop

If the vial holds semaglutide, tirzepatide or retatrutide bought outside a prescription, testing is the wrong step. Those three sit under an active import alert and an enforcement campaign. The dosing-error record is concentrated in exactly that group, and a clean purity number would not make self-injecting them safer. Approved versions exist, and a prescriber can talk you through them.

What Would Change This Answer

Legal compounding access for these compounds would change it. Move the peptides people currently buy as research chemicals onto a lawful prescription route, and the sterility and dose-accuracy obligations shift to a licensed pharmacy. Mandatory per-lot testing would change it too. Suppose sellers had to publish identity, quantity, sterility and endotoxin results for every lot, in a database a buyer could search. The verification work would move off the buyer entirely.

Until either happens, treat gray market peptides as a batch you cannot vouch for. Check the lot number against the certificate and verify that certificate at the issuing laboratory. Take what you find to a clinician before the next dose.
