Early Study Suggests Hormone Combo May Help Menopause Brain Fog

A new observational case series from UCLA Health suggests that a hormone treatment combining estriol and progesterone may help reduce cognitive symptoms such as memory lapses and trouble concentrating in menopausal women. The study followed 20 women with an average age of 53 over a 12-month period.

Participants received a customized regimen of estriol and progesterone, and reported improvements in brain fog, concentration, working memory, processing speed, verbal memory, and problem-solving compared to their baseline symptoms.

The study, published in Scientific Reports, was an observational case series rather than a randomized controlled trial. It included 20 menopausal women who were assessed for cognitive symptoms before and after a year of hormone treatment.

The findings are based on self-reported symptom changes and do not include a placebo group or randomization, so results may not apply to all menopausal women.

How Estriol Differs From Other Estrogen Therapies

Estriol is a natural form of estrogen produced during pregnancy and is used in Europe and Asia for menopausal symptoms. Unlike estradiol, the most common estrogen in U.S. hormone therapy, estriol binds primarily to a different estrogen receptor in the brain.

Previous research has suggested estriol may help protect brain cells and reduce atrophy in the hippocampus, a region important for learning and memory.

To explore how the treatment might work, the UCLA team also studied estriol in midlife female mice. In these animals, estriol reduced markers of brain pathology in the hippocampus and improved working and spatial memory.

These findings suggest a possible biological mechanism for estriol's effects, but animal results do not always translate to humans.

Limitations and Next Steps for Research

The study's authors caution that the results are preliminary due to the small sample size, lack of placebo group, and absence of randomization. The hormone treatment is patented by UCLA and licensed to CleopatraRX, with a study author serving as a medical advisor.

Larger, placebo-controlled clinical trials with brain imaging and standardized cognitive testing are needed to confirm whether estriol and progesterone can reliably improve cognitive symptoms in menopause.
