Phase 2 Trial Assesses Anti-IL-6 Antibody in Chronic Kidney Disease

The TRANQUILITY phase 2 clinical trial evaluated pacibekitug, an investigational monoclonal antibody targeting interleukin-6 (IL-6), in adults with chronic kidney disease (CKD) and high inflammation-related cardiovascular risk.

The study enrolled 143 adults with CKD and elevated high-sensitivity C-reactive protein (hsCRP), a marker of inflammation. Participants received either placebo or one of three pacibekitug dosing regimens and were followed for 180 days.

Sustained Reductions in Inflammation Biomarkers Observed

Pacibekitug treatment resulted in substantial and sustained reductions in hsCRP, a key inflammation biomarker, through day 180. Time-averaged hsCRP reductions ranged from 76% to 89% across all pacibekitug groups, compared to a 7% increase in the placebo group.

The trial also found favorable effects on other biomarkers associated with IL-6 pathway activity and cardiovascular risk. These findings suggest durable suppression of the IL-6 pathway is possible with infrequent dosing of a long-acting monoclonal antibody.

Study Design and Applicability

TRANQUILITY was a randomized, double-blind, placebo-controlled study, considered the gold standard for clinical research. The results apply to adults with chronic kidney disease and elevated hsCRP, a group at increased risk for cardiovascular disease.

Earlier analyses showed reductions in inflammatory biomarkers through 90 days; this final analysis extends those findings to 180 days, indicating sustained effects.

Practical Context for Metabolic Health Management

Inflammation is increasingly recognized as a contributor to cardiovascular disease, especially in people with chronic kidney disease. High hsCRP levels are linked to higher cardiovascular risk.

Pacibekitug was generally well tolerated, with no new safety signals, but longer-term safety and efficacy are not yet known.
