AMH Fertility Tests Cannot Predict Natural Pregnancy
A ten-year study of 1.4 million Australian women finds anti-Müllerian hormone testing rose 13-fold despite poor predictive value for natural conception.
The Surge in Anti-Müllerian Hormone Testing
Requests by general practitioners (GPs) for anti-Müllerian hormone (AMH) blood tests increased more than 13-fold in Australia between 2011 and 2021. Researchers from Flinders University analyzed medical records from nearly 1.4 million Australian women to track testing trends over that ten-year window. The retrospective cohort study was published in the Australian and New Zealand Journal of Obstetrics and Gynaecology.
The sharpest increase occurred among women aged 30 to 34, whose testing rates jumped almost 20-fold across the decade. Study authors noted that the test is widely marketed as an egg count check to evaluate personal fertility potential. This commercial push, combined with later childbearing and social media discussions, fueled the rapid adoption.
What Anti-Müllerian Hormone Tests Measure
An anti-Müllerian hormone (AMH) test measures ovarian reserve but does not evaluate egg quality or predict spontaneous conception. The circulating hormone reflects the estimated quantity of remaining eggs inside the ovaries. It cannot tell a healthy patient whether she will conceive naturally, how long conception will take, or how many fertile years remain.
Lead author Associate Professor Luke Grzeskowiak of the Flinders Health and Medical Research Institute (FHMRI) noted that public assumptions do not align with clinical evidence. A normal result does not guarantee future fertility if other reproductive factors are present. Similarly, low levels do not mean that a patient is infertile.
Test Fluctuations and the Risk of Misleading Results
Roughly one-third of women who retested after an initial low reading shifted into the normal reference range. Across the study cohort, nearly one in five women received a result categorized as low. The researchers cautioned that unexpected low readings often trigger unnecessary anxiety and hasty life decisions.
Age remains the single most reliable predictor of pregnancy odds and egg quality. Relying on an isolated hormone level to guide relationship timelines, family planning, or elective egg freezing can lead to unhelpful interventions. Clear clinical guidance helps prevent false reassurance from normal scores and unwarranted alarm from low numbers.
Testing Disparities and Out-of-Pocket Costs across Australia
Access to anti-Müllerian hormone (AMH) testing varies according to patient location, household income, and individual clinic habits. Testing rates were higher in major metropolitan areas and economically advantaged zip codes. Women holding government concession cards were far less likely to receive a test requisition.
The test is not subsidized by Medicare in Australia and typically costs more than $100 out of pocket. Practice patterns also created large divergences, with more than 10 percent of surveyed clinics ordering zero tests during the entire decade. Practice-level preferences accounted for roughly 21 percent of the overall variation in testing rates.

Anti-Müllerian hormone (AMH) tests provide useful guidance in specialist fertility clinics rather than general population screening. Reproductive endocrinologists rely on the marker to predict ovarian response during in vitro fertilization protocols and dosage planning. It is not designed to screen healthy individuals who are trying to conceive without medical assistance.
Patients considering reproductive blood work should discuss their goals with a qualified healthcare provider. Comprehensive evaluations of ovulatory cycles, structural factors, and partner health offer a clearer picture than a standalone blood draw.
