A low MCV means your red blood cells are smaller than average. Here is what causes it, what it means when hemoglobin is still normal, and how the cause is identified.

Low MCV (mean corpuscular volume below about 80 fL) means your red blood cells are smaller than normal — called microcytic. When MCV is low but hemoglobin is still normal, the two most common explanations are early iron deficiency (before anemia develops) and thalassemia trait. Iron studies, RDW, and the Mentzer index help tell them apart.

A low MCV with normal hemoglobin usually points to one of two things: early iron deficiency or thalassemia trait. MCV (mean corpuscular volume) measures the average size of your red blood cells in femtoliters (fL). When it drops below roughly 80 fL, the cells are smaller than normal, a state called microcytic. Hemoglobin (Hb), on the other hand, measures the amount of oxygen-carrying protein in your blood. It is entirely possible for red blood cells to shrink before hemoglobin drops. That is why MCV can be low even while Hb remains in the normal range.

In early iron deficiency, the body doesn't have enough iron to make full-sized red cells, so it begins producing smaller ones. Hemoglobin can remain normal for weeks or months before iron-deficiency anemia develops, which means a low MCV can be an early warning sign. With thalassemia trait (also called thalassemia minor), an inherited difference in how hemoglobin is made causes the body to produce many small red cells from birth, often with little or no anemia.

What Is MCV?

MCV (mean corpuscular volume) reports the average size of your red blood cells. It is a standard part of the complete blood count (CBC) and is measured in femtoliters (fL). A small MCV means small cells; a large MCV means large cells. The result is one of the first clues doctors use to sort out the cause of an abnormal blood count.

Reference ranges vary slightly between laboratories and can differ a little by age and sex, so always read your MCV against the range printed on your own report. A value just outside the range is common and is interpreted alongside the rest of the CBC rather than on its own.

Low MCV With Normal Hemoglobin

When MCV is low but hemoglobin (Hb) is normal, the body is making smaller red cells without yet showing anemia. Two causes account for the large majority of these results.

Early iron deficiency. Iron is required to build hemoglobin and full-sized red cells. As iron stores fall, the bone marrow begins turning out smaller cells first — so MCV can drop while hemoglobin holds steady for a time. This is why a low MCV with normal Hb is often the earliest laboratory sign of iron depletion, appearing before iron-deficiency anemia develops. Common drivers include low dietary iron, heavy menstrual periods, pregnancy, or slow blood loss from the digestive tract.

Thalassemia trait (thalassemia minor). This is an inherited condition in which the genes for hemoglobin produce slightly less of one of its protein chains. People with the trait make many small red cells from birth, so MCV is characteristically low — often quite low — while hemoglobin is normal or only mildly reduced. It is usually harmless to the carrier, but it matters for family planning and is important not to mistake for iron deficiency, since iron supplements do not fix it.

Less commonly, a low MCV can accompany anemia of chronic disease (long-standing inflammation or infection), lead poisoning, or sideroblastic anemia. These are rarer and are sorted out by a physician using additional testing and your history.

How to Tell the Cause Apart

The key step is checking iron studies, especially ferritin, which reflects the body's iron stores.

  • Low ferritin points strongly to iron deficiency — the iron tank is running empty.
  • Normal (or high) ferritin with a low MCV makes iron deficiency unlikely and raises suspicion for thalassemia trait, which is then confirmed with hemoglobin testing (such as hemoglobin electrophoresis) and sometimes genetic testing.

Two more clues from the CBC help before any extra blood is drawn:

RDW (red cell distribution width) measures how much red cell sizes vary. In iron deficiency, new small cells mix with older normal-sized ones, so RDW is usually high — the pattern behind the search ‘low MCV high RDW.’ In thalassemia trait, cells are uniformly small from the start, so RDW is often normal.

The Mentzer index divides MCV by the red blood cell count (RBC). A result greater than 13 leans toward iron deficiency, while a result less than 13 leans toward thalassemia trait (people with the trait tend to have a high red cell count despite small cells). The index is a helpful pointer, not a diagnosis — ferritin and hemoglobin testing confirm the cause.

Low MCV With Low MCH or MCHC

Low MCV often appears with low MCH or MCHC in iron deficiency and thalassemia trait. Mean corpuscular hemoglobin (MCH) is the average amount of hemoglobin inside one red blood cell, reported in picograms (pg). Mean corpuscular hemoglobin concentration (MCHC) shows how concentrated that hemoglobin is within the cell, reported in grams per deciliter (g/dL). MCHC describes how full the cell is with hemoglobin, independent of its size. Small cells with less hemoglobin per cell are called hypochromic, or pale, which gives rise to the term “microcytic, hypochromic anemia.”

The MCHC pattern can help distinguish the two common causes. In iron deficiency, MCV, MCH, and MCHC are typically low because the cells are small and contain diluted hemoglobin. In thalassemia trait, MCH is also typically low, while MCHC is often normal or only mildly reduced. The cells remain small but are relatively well-packed with the hemoglobin they make. A clinician interprets low MCH or MCHC alongside the full complete blood count (CBC), rather than using either result alone.

When to See a Doctor

A low MCV is a flag for follow-up, not an emergency on its own. Share the result with a clinician if your report shows a low MCV, particularly when paired with a high RDW, a low or borderline hemoglobin, or symptoms such as fatigue, weakness, shortness of breath, pale skin, or unusual cravings (for example, for ice).

Seek prompt medical attention if a low MCV comes with signs of significant blood loss — black or bloody stools, heavy or prolonged menstrual bleeding, or marked breathlessness — since these can point to a source that needs treatment. A clinician will typically check iron studies and may repeat the CBC, review your diet and medical history, and decide whether testing for thalassemia or other causes is warranted. Avoid starting iron supplements on your own based on a low MCV alone: if the cause is thalassemia trait, iron will not help and excess iron can be harmful.

Should I take iron if my MCV is low?

Not without checking first. Iron supplements help only if the low MCV is due to iron deficiency, confirmed by a low ferritin. If the cause is thalassemia trait, iron will not raise the MCV and taking extra iron unnecessarily can be harmful by causing iron overload. Ask a clinician to check iron studies before starting any supplement.

Is a low MCV normal during pregnancy?

A low MCV is not usually a normal, harmless change during pregnancy. Pregnancy raises iron needs to support maternal red blood cell expansion, the placenta, and fetal growth. Iron deficiency is therefore a common cause of anemia and low MCV while pregnant. This differs from pregnancy's dilutional effect, which lowers hemoglobin and hematocrit as plasma volume rises faster than red cell mass but does not typically lower MCV. Prenatal blood counts help detect this, and a clinician will often check iron studies such as ferritin before recommending iron supplements. Ask your clinician to interpret a low MCV rather than self-diagnosing.

Can a low MCV be a sign of cancer or leukemia?

Cancer or leukemia is not a common or typical cause of an isolated low mean corpuscular volume (MCV). By far, iron deficiency and thalassemia trait are the two most common causes. Leukemia and lymphoma usually appear on a complete blood count (CBC) with other abnormalities, such as unusual white blood cell or platelet counts. An indirect link is possible because some cancers in the gastrointestinal tract can cause slow, hidden blood loss and iron-deficiency anemia. Doctors may investigate unexplained iron-deficiency anemia further, particularly in adults without heavy periods or another clear source. Ask a clinician to interpret your results.
