For menstruating women, when you test a hormone matters as much as which hormone you test, because levels of FSH, LH, estradiol, and progesterone shift predictably across the menstrual cycle. Testing on the wrong day can produce a result that looks abnormal but is actually just a normal value for a different phase of the cycle — or vice versa.

Why Is Day 3 the Standard for FSH, LH, and Estradiol?
Day 3 of the menstrual cycle (counting the first day of full flow as day 1) falls in the early follicular phase, before the dominant follicle has started producing significant estrogen. At this point, follicle-stimulating hormone (FSH) and luteinizing hormone (LH) reflect the pituitary's baseline signal to the ovaries, largely uninfluenced yet by feedback from a maturing follicle. Estradiol should still be relatively low at this stage.
This baseline snapshot is traditionally used in fertility-adjacent evaluation because an elevated day-3 FSH (often flagged above roughly 10 mIU/mL, though exact cutoffs vary by lab and assay) can suggest the ovaries are requiring a stronger pituitary signal to recruit a follicle — a pattern clinicians associate with diminished ovarian reserve. A high day-3 estradiol can also make FSH look artificially normal by suppressing it, which is why the two are usually interpreted together rather than in isolation.
Testing FSH, LH, or estradiol later in the cycle — say, near ovulation, when LH surges and estradiol peaks — produces values that are expected to be much higher and would be misread as abnormal if compared against day-3 reference ranges. That's the core reason cycle day is specified whenever these three hormones are ordered together.
Why Is Progesterone Tested Around Day 21 Instead?
Progesterone works on the opposite timing logic. Its job in the cycle is to rise after ovulation to prepare and maintain the uterine lining, so it is essentially uninformative early in the cycle — there's very little to measure until the corpus luteum starts producing it post-ovulation. Clinicians typically test 7 days after presumed ovulation, which lands around day 21 in a standard 28-day cycle (ovulation around day 14, plus 7 days).
This is sometimes called a "mid-luteal progesterone" and is one of the more accessible ways to get indirect evidence that ovulation occurred, since progesterone should only rise substantially if a corpus luteum formed. A low result at the expected mid-luteal point can prompt further evaluation, but a single low value isn't automatically diagnostic — timing errors (testing too early or too late relative to actual ovulation) are a common and correctable cause.
Because ovulation timing varies by cycle length and from person to person, "day 21" is a convenient shorthand for a 28-day cycle rather than a fixed rule. Someone with a 35-day cycle would generally be tested later (roughly day 28), and someone with a shorter cycle earlier — ideally based on ovulation predictor kits, basal body temperature tracking, or ultrasound timing rather than the calendar date alone.
How Do Birth Control and HRT Confound Hormone Testing?
This is the single most important caveat in cycle-based hormone testing: hormonal birth control and hormone replacement therapy (HRT) suppress the body's natural hypothalamic-pituitary-ovarian (HPO) axis signaling. Combined oral contraceptives, the patch, the ring, and hormonal IUDs all work in part by flattening or overriding the natural FSH/LH surge pattern that drives ovulation. Testing FSH, LH, estradiol, or progesterone while on these methods largely measures the effect of the medication, not your underlying ovarian function.
In practice, this means:
- FSH and LH will typically read low and non-cyclical on hormonal birth control, since the pituitary isn't receiving the normal feedback signals
- Estradiol and progesterone levels will reflect whatever synthetic or bioidentical hormones are in the product, not natural ovarian production
- A "day 3" or "day 21" label becomes largely meaningless while a suppressive method is active, because there is no natural cycle driving those numbers

Irregular cycles make calendar-based timing ("day 3," "day 21") less reliable, since these windows assume a roughly predictable cycle length and ovulation date. In this situation, a physician may take one or more of these approaches instead:
- Random-day testing for certain hormones (like TSH or prolactin) that aren't strongly cycle-dependent
- Serial testing across several days or weeks to capture a pattern rather than a single snapshot
- Ultrasound tracking or ovulation predictor kits to pinpoint ovulation before timing a luteal progesterone draw
